The Sunderland Health and Wellness Hub has been built by local healthcare professionals. Dr Raj Bethapudi, a local GP and Project Lead, said: “Digital technology is transforming the way we do everyday things, including accessing health and care services.
“Currently, information about local services can be found in lots of different places on the internet. This can make it difficult for patients to find. By creating the Sunderland Health and Wellness Hub, we can put all of this information in one place to create a local directory of services.
“The Hub provides a quick and easy way to find information about local services. People can use the site to refer themselves to services or find reliable information for a range of different conditions including mental health and physical ailments.
“People can also use the site to self-refer to the Social Prescribing Team. Social prescribers can help people with non-medical problems like; isolation, housing worries and other care needs. This can be done without needing to contact a GP practice, meaning people see the right professional for their needs, first-time.”
Paul Gibson, Head of Digital Development at North East and North Cumbria Integrated Care Board, said: “The Hub is an excellent example of collaborative working across GP practices to build, contribute to, and adopt a city wide solution that benefits patients.
“Patients can access the Hub directly or they may be signposted to it by a member of the practice team. In addition to using the NHS App, we hope the Hub becomes place to look for health and care information when accessing general practice digital services.
“The vision going forward is to develop the Hub as the ‘digital front door’ for patients. By working with health and care partners and patients we want to improve the content that is available.”
